Each anchor holds its own clinical license — and its own PHI.Atlas Wellness Holdings, LLC is a holding company. It does not deliver clinical care, does not bill insurance, and does not hold protected health information about individuals. The clinical entity that admits you — your anchor — is the HIPAA-covered entity for your care. This page describes the federation-level framework. Your anchor's Notice of Privacy Practices applies to your individual record.
1. Who is covered
The following entities are the licensed clinical providers in the Atlas federation, each operating as an independent HIPAA-covered entity:

Each anchor publishes its own Notice of Privacy Practices, available at intake and on its individual website.
2. Atlas's role
Atlas Wellness Holdings provides the following to each anchor under Business Associate Agreement (BAA):
- The Atlas Arc engagement-and-outcomes platform (clinical-side data)
- Atlas Integrated Health, Wellness, Therapy, and Living service subsidiaries
- Cross-anchor outcomes aggregation (in de-identified form for research, in identified form only when a patient consents to step across anchors)
Each BAA limits Atlas's use of PHI to the activities described — and specifically prohibits any commercial use, sale, or non-treatment disclosure of identifiable data.
3. How PHI flows
- Within your anchor: PHI is shared with the clinical team treating you, consistent with HIPAA's "treatment, payment, healthcare operations" purposes.
- Across the federation: PHI is shared with another Atlas anchor only with your specific written consent — for example, when stepping down from NIRC residential care to Millennium outpatient. The Arc accompanies you only when you consent for it to.
- With Atlas Holdings, LLC as Business Associate: limited PHI flows to the Arc platform for engagement tracking and outcomes — under BAA, never for commercial purposes.
- De-identified data: used for outcomes research and curriculum improvement. De-identification follows the HIPAA Safe Harbor standard (45 CFR § 164.514).
4. What we do not do
- We do not sell PHI. The Atlas BAA structure prohibits it absolutely.
- We do not use PHI for advertising or marketing communications outside of treatment.
- We do not share PHI with employers, schools, or law enforcement without your authorization, a valid subpoena, or a clear legal mandate.
5. Your rights
Under HIPAA, you have the right to:
- Inspect and obtain a copy of your record
- Request an amendment to your record
- Request a list of disclosures (an "accounting")
- Request restrictions on certain uses and disclosures
- Request confidential communications (e.g., by mail rather than phone)
- File a complaint with your anchor's Privacy Officer or directly with HHS Office for Civil Rights

6. Substance use disorder records · 42 CFR Part 2
Records concerning substance use disorder treatment at an Atlas anchor are additionally protected under 42 CFR Part 2, which is stricter than HIPAA. These records cannot be disclosed without specific written consent except in limited emergency, audit, or court-order circumstances. Each anchor maintains the heightened consent protocols required by Part 2.
7. Breach notification
If a breach of unsecured PHI occurs, the affected anchor (as covered entity) and Atlas (as business associate) will notify affected individuals within 60 days of discovery, consistent with the HIPAA Breach Notification Rule. Significant breaches are also reported to HHS and, where required, to the media.
